Appendix 4417. Lost license tag.

If a license tag issued under the provisions of this Article is lost, a duplicate tag may be obtained from the Division upon paying a fee in the amount prescribed by resolution of the Board of Supervisors and by either surrendering the receipt obtained for the lost license tag or certifying under penalty of perjury that a tag was obtained and that the receipt cannot be found.
